pygittools,pgt: less aggressive model for repo scanning and refresh()
* keystrokes that move the cursor or edit text no longer trigger rescan
* collapse and expand use a cache rather than rescan
* log reloads on up/down but no other keys (this may need a revisit but is an improvement)
